Sports Medicine Agreement
Dynamix Physical Therapy, LLC (herein called "Dynamix") and
(herein
called "Client") are hereby entering into a Sports Medicine Agreement (herein called "Agreement") as of
, 20 .
Upon execution of this Agreement, Dynamix shall become the exclusive sports medicine director of the
Client.
Any medical sideline/court/field personnel will be cleared by Dynamix DOSM Ross Pope and meet
Dynamix approved criteria for direct care of athletes. It is agreed that medical providers not
authorized by Ross Pope, DOSM will not be permitted access to stand on sideline/court/field for
treatment or advertising purposes. Dynamix will provide Client with notification of any approved
additions to the Dynamix Sports Medicine team.
Under the terms of this Agreement, the Client shall receive the following sports medicine services:
High School Football
Blanket coverage for all varsity games (home, and playoff)
High School Basketball (Boys/Girls)
Blanket coverage for all varsity home and playoff tournament game
All other sports
On-call access
Weekly well checks with coaches (Sports "in season" only)
Under the terms of this Agreement, Dynamix shall receive the following advertising/marketing opportunities:
Branding of the Athletic Courts and Fields with Dynamix Logo (where applicable)
In game recognition (Public address, radio, video scoreboard, or other applicable medium) as being
the sports medicine partner of the client
Term: The term for this agreement shall be three (3) years from the date of execution.
Termination of Agreement: Upon the early termination of this Agreement by the Client, notification of intent
to terminate this agreement shall be done in writing and sent to the mailing address listed below:
